Conditional Failed Status Email Template?

How could one write an email template for sabnzbd that is only sent out if the download fails?

Have you read this: http://wiki.sabnzbd.org/email-templates
The variable you should test is $status
The example at the bottom tests for the category, but that can easily be changed.

BTW: The standard email notification can be set to "error only".
